Payment Setup
How billing works: When you submit this form, a Credit Card Authorization form will automatically download to your device. Complete it and return it to operations@agapechildcarestaffing.com — or call us at (214) 225-0039 to provide card details securely by phone. Please do not email full card numbers unencrypted. Your monthly membership is charged on the 1st, and caregiver hours are billed biweekly from approved timesheets.
